Please note that you will only be placed on the wait list for the month you requested. Once that month has lapsed, and you have not been called it is your responsibility to either be placed back on for the next month or use the online booking portal to book an appointment.
The wait list is cleared at the beginning of each month, and online booking opens up in 3 month intervals on a first come first served basis.

Please be aware that depending on how your insurance coverage is set up, payment may only be made to the insurer (you) and be declined to the provider (me). In this case you will have to pay at the time of the appointment and recover the costs from your insurance company.
